NutraNom Privacy Policy

At NutraNom, we’re committed to safeguarding your personal information as we help you on your nutrition journey. You receive the best guidance from us when you are comfortable sharing your information. We recognize the trust you place in us, and your privacy will be respected and protected.

 

This Policy explains how we collect, use, and share your personal information. Your privacy is protected under the Personal Health Information Protection Act (Ontario) and the Personal Information Protection and Electronic Documents Act (Canada). What is Personal Information?

 

Personal information is any information about an identifiable individual (“PI”) such as name, contact details and date of birth. PI includes personal health information which is information about your:

 

  • Medical/health issues, diagnoses and concerns (physical and mental)

  • Medications and  treatment protocols

  • Physical condition and social situation

  • Economic information (relating to food affordability)

  • Level of stress, anxiety depression, and activities that impact your health

  • Family health history


How does NutraNom Collect, Use, and Share my PI?


We collect PI directly from you, when you register as a client, when you complete our intake and consent forms (or when we input them on your behalf), when we speak with you, and through our observations during appointments and home visits. We may also collect PI from your family members, if they are with you during an appointment or visit. We may gather PI from your physician or other health care provider, with your consent, or to the extent PI has been shared as part of a referral

 

We store PI using appropriate technical and organizational measures to keep it secure, and it is only available to the Registered Dietitian you are working with. We will maintain records of sessions for clinical and legal purposes. You have the right to request access to your records, in accordance with applicable privacy laws.

We use your PI only to:

 

  • provide nutrition services to you and your family;

  • communicate with you, schedule visits, and collect payment;

  • satisfy our legal and regulatory obligations; and

  • manage our practice and operations, including internal research, benchmarking and quality assurance.


If sessions are conducted via phone or video, and as we communicate over email or text, all reasonable efforts will be made to ensure privacy and confidentiality. However, there are inherent risks in electronic communication that cannot be entirely eliminated, and you assume these risks.


When we conduct a home visit, we may capture photographs and video which will be used to support you and may also be shared on social media or in group presentations. The photographs will never capture the likeness of any person nor identifying elements of your home or your family.


While we take appropriate technical and organizational measures to protect your PI, including using practice management software, Practice Better, with privacy and security protections, unforeseen events can occur and there remains a risk that your personal information will be disclosed. 


If you are working with us as a family, we will meet separately with the child and the parent(s) and also meet together. Since nutrition is a family conversation, we will share your information within the family, unless you instruct us not to. Please speak to us if this is important to you. We will not share your PI with anyone else, unless permitted or required by law. For example, we must disclose PI if there is suspected abuse, and we may disclose PI if we believe on reasonable grounds that disclosure will eliminate or reduce a significant  risk of serious bodily harm to you or another.

We also use your PI to aggregate data for insights, trends and benchmarking. None of this aggregated data identifies you.

How Long Does NutraNom Keep my PI?

We keep PI after you have worked with us so that we have a record of our relationship, and to meet our regulatory obligations. The retention period is at least 10 years after our last interaction or after a client turns 18.

 

When we destroy PI, we ensure that the deletion is secure and permanent.

 

Accessing, Updating and Deleting Your PI

 

You have the right  to understand what PI we hold about you and with whom we have shared it. Should you wish to access your information, or to update anything you feel is no longer correct, please contact us tracy@nutranomnutrition.ca.

 

Similarly, if you would like to delete your PI, please contact us. We will remove it subject to our need to comply with any legal or regulatory requirements.


Privacy Incidents


NutraNom will take appropriate steps to contain, investigate, and remediate data security incidents. We will notify applicable regulatory authorities and affected parties about data security incidents as appropriate and/or required by law and will aid any applicable authorities in the management and handling of such data security incidents.

 

Updates to this Policy


Changes to this Policy from time to time will be shared on this site. We also use your PI to aggregate data for insights, trends and benchmarking. None of this aggregated data identifies you.
